In school we all learned the acronym SOAP for Subjective, Objective, Appraisal, and Plan. The insurance industry (especially Medicare) has been moving in the direction of requesting additional information in the form of extra appraisal data. Marty Kotler, DC, of Target Coding calls this the "extra A." An example of this is to state noticeable improvement (or lack thereof) by describing that the patient was initially unable to put on his socks and shoes due to the pain of a lumbar disk herniation, but after several treatments, this is no longer a problem. The great thing is that by using the story button by EZ Notes, Chicago, this can easily and quickly be accomplished. The story button can be used for the "mechanism of injury" (on the first visit) as well as the "extra A" (on additional visits). When the patient first presents to the office, we pick the first visit button, then the body part, then the story button stating something like, "He hurt his back when he tried to pick up a piano by himself," then we give the verbal analog scale (ie, eight out of 10). When the patient returns on a subsequent visit, we can say "slightly better"; and using the story button, we can add, "It no longer hurts to bend over the sink to brush his teeth," and reduce the VAS to five out of 10. We may even keep the VAS the same, but note some slight improvement.
